What the callback form asks for

The form asks for contact details, a general interest category, a preferred callback time, and permission for a general voicemail. Even without medical details, contact with a healthcare practice can be sensitive. Do not enter symptoms, diagnoses, records, or information about other people.

How submission works

In this build, submissions are disabled until the site operator configures an approved receiving endpoint. The server does not save form contents to a file or database and does not print them to application logs. If enabled, information is sent to the configured receiving service. That service’s access controls, retention, security, and applicable agreements must be reviewed before launch.

External photographs and fonts

Photos load from Pexels, and typography may load from Google Fonts. These requests disclose technical connection data, including an IP address, to third parties. No advertising pixels or analytics are included in this build. Review whether externally hosted assets are appropriate before public launch.

Phone and email

Ordinary email should not be used to send health information. Call the office to obtain an approved secure channel for clinical records. The public contact form is not suitable for clinical referrals.

Before launch

The operator must approve hosting, privacy notices, data handling, retention, access, applicable BAAs, accessibility, consent language, and the clinical communication process. This build does not claim HIPAA compliance.
